Anti-Wrinkle Injections vs Dermal Fillers: Which Do You Need?

Anti-wrinkle injections relax the muscles behind expression lines; hyaluronic acid fillers restore volume and contour. How to tell which a line needs, how each works, how long they last, and when both make sense.

Anti-wrinkle injections and dermal fillers solve different problems. Anti-wrinkle injections (neuromodulators) temporarily relax the small muscles that crease the skin, so they suit lines that appear when you frown, raise your brows or smile. Dermal fillers made from hyaluronic acid add volume and support, so they suit hollows, folds and contour. Many faces need one, some need both, and a physician assessment decides.

Below is a side-by-side comparison, then the questions people ask most when choosing between them. Prices are not covered here; your plan and cost are explained at consultation.

How do anti-wrinkle injections and dermal fillers compare at a glance?

Anti-wrinkle injections act on muscle movement, while fillers act on volume and structure, so they differ in what they treat, how fast they work and how long they last.

How do anti-wrinkle injections work?

A neuromodulator temporarily blocks the signal from nerve to muscle, so an overactive muscle rests and the skin over it creases less.

Dose is tailored to each muscle's strength rather than sold as a fixed package, because too much in the wrong place causes heaviness or a "frozen" look. Treated early, relaxing a muscle can help stop expression lines etching deeper over time. The same principle is used to relax an enlarged masseter (jaw) muscle, which can soften a square jaw and ease clenching-related tension. Products are sourced through Canadian suppliers, and you can ask which one is used for your plan.

How do dermal fillers work?

Hyaluronic acid fillers are gels injected to restore volume, soften folds and shape contours, using a substance the body naturally contains.

Cross-linking makes the gel smoother and longer-lasting, and different formulations suit different jobs: firmer gels for structure in the cheeks and chin, softer ones for lips and fine lines. Your physician chooses the product and depth for each area. RAAC uses genuine hyaluronic acid filler products. Hyaluronic acid filler can be dissolved with hyaluronidase if needed.

Which concerns lean toward anti-wrinkle injections, and which toward fillers?

A line that shows only when you move, or is deepest when you frown or squint, leans toward anti-wrinkle injections; a line or hollow that is there at rest because volume has been lost leans toward filler.

A simple check is to look in the mirror with a relaxed face, then frown and smile. Forehead lines, the 11s and crow's feet are usually dynamic. Nasolabial folds, marionette lines, hollow temples and tear troughs are usually about volume and support. Deep, long-standing lines can be a mix, which is where combined plans come in. Neither gives surgical-level lifting.

What does each treatment feel like?

Anti-wrinkle injections feel like a few quick pinches with an ultra-fine needle, while fillers feel more like pressure with brief stings, eased by numbing cream and lidocaine.

Most people find both well tolerated. Filler appointments take longer because the physician shapes and checks the result as they go.

How soon do results show, and how long do they last?

Fillers show on the day and typically last 9 to 18 months, while anti-wrinkle injections take a few days to start, peak at about 2 weeks and typically last 3 to 4 months.

Masseter treatment can last 4 to 6 months. A 2-week check may be suggested after anti-wrinkle injections to fine-tune the dose. Duration varies with the product, area, dose, muscle strength and metabolism.

What are the common reactions and recovery?

Most people resume normal activities the same day after either treatment; temporary redness, small bumps, swelling or bruising are possible.

After anti-wrinkle injections, stay upright for 4 hours, avoid pressing or massaging the area, skip strenuous exercise, sauna and hot yoga for 24 hours, and avoid facials, lasers or microcurrent on the treated zones for 7 days. After fillers, avoid touching or massaging the area for 24 hours, skip saunas, hot baths and strenuous exercise for 48 hours, and avoid makeup for 24 to 48 hours. Rarely, filler can block a blood vessel: severe pain, whitening or colour change of the skin, or vision changes mean you should contact us or seek emergency care right away.

Who should not have either treatment?

Pregnancy, breastfeeding and active infection or irritation at the injection site rule out both, and each has its own allergies and conditions to check.

  • Anti-wrinkle injections are not suitable with a known allergy to the product or its ingredients, or a neuromuscular condition such as myasthenia gravis unless cleared by your physician.
  • Dermal fillers are not suitable with an allergy to hyaluronic acid or lidocaine, recent facial surgery or filler, or autoimmune or bleeding disorders without review.

Can they be combined?

Yes, many people have both: neuromodulators relax movement while fillers restore volume, and your physician maps the combination for a natural result.

A common pairing is relaxing the frown muscles while restoring cheek support, so the lower face is not carrying the work alone. Either can also sit alongside skin-quality treatments such as Sylfirm X or PicoWay. The order and spacing are set at consultation.

How is the choice made at RAAC?

At RAAC in Richmond, BC, the choice is made at a physician assessment with Dr. Charles Jiang, who studies your expressions, muscle strength and volume at rest before recommending one, the other or both.

Planning is proportion-led, aiming for you to look rested rather than obviously "done". Consultations are available in English, Mandarin and Cantonese at Unit 110, 5580 No. 3 Rd, about a 3-minute walk from Lansdowne Station on the Canada Line, open every day from 9:00 to 18:00. Your physician explains the plan and cost before any treatment.

Frequently asked questions

Can anti-wrinkle injections fill a deep fold?

No. They relax muscles; they do not add volume. A fold that is present at rest because of volume loss is usually a filler question.

Can filler smooth forehead lines?

Fine lines at rest are sometimes softened with a soft filler, but lines caused by muscle movement usually respond better to anti-wrinkle injections. Many plans use both.

Will I look frozen or overfilled?

Not when dose and volume are matched to your face. A frozen look comes from over-dosing or poor placement, and an overfilled look from too much product; conservative planning avoids both.

Can filler be reversed?

Hyaluronic acid filler can be dissolved with hyaluronidase if needed. Anti-wrinkle injections wear off on their own over months.

How often will I need treatment?

Anti-wrinkle injections are usually repeated about every 3 to 4 months, and fillers every 9 to 18 months, depending on area and product.

Can I have both on the same day?

Often, yes, if your physician agrees it suits your plan. Sometimes they are spaced so each result can be judged on its own.
